The HIV voluntary form are usually given to clients or patients who want to undergo HIV Screening Test. Ideally HIV testing should be carried out on social hygiene clinic and government run hospital who are participating in such event.
If you missed the Free testing week the test can be done at DOH Accredited Clinical Laboratories and can be carried out in a private stand alone laboratory or in private or government run hospital capable of performing such tests and have licenses to perform such test including their laboratory personnel (Medical Technologist / Clinical Laboratory Scientist). However, one should pay for first this test.
A few years ago, I was assigned to interview a patient. He filled out his HIV voluntary testing forms in a subtle manner. As I am looking at him he seems to be fairly normal guy to be I am really accustomed with the "LOOKS ARE DECEIVING" type of scenario because I did have small conversation with this good-looking young guy whom I doubt to be a "bisexual" but it turns out my hunch was correct after initial interviews and his disclosure about his sexuality in which he was embarrassed to tell me as a healthcare professional. I was looking out his forms before proceeded reading it and find out the information he gave was correct in which he had sex with both male and female and had previous unattached multiple sexual encounters in the past. This good looking bi-guy is more sexually active than me. I felt aghast to learn such information as this is all confidential and against the HIV laws imposed by senate and congress in 1998.
My concluding thoughts
Somehow, HIV / AIDS in the Philippines has become fast and furiously spreading in recent years with careful reporting of DOH and other non-government support groups. However, as I see it in my healthcare standpoint, Pinoys are "PASAWAY" (troublesome and stubborn in the English language) that they disregard they health and safety of themselves and the others instead living the moment with their libido first. Hope their mindset change now.
